At teams partnership should probably play 4H. While West's 1NT rebid is "offshape" it is a good bid, better than the alternative 3D. East should consider a slam invite, but reject it and just bid 4H. Consider a "perfect minimum" SK, HA, DA, CA - there appears to be the SA loser plus 2 club losers (which may of course find a home if pard has suitable shape etc).
What to lead. The CJ looks odd and could cost a trick. There is a bias towards the unbid major on such hands. Indeed, DDA shows this with a 0.1 av. trick advantage over the next best (for MPs) and a 2% advantage over the next best (low club), with a 8% advantage over the other suits. The CJ was about 6% worse than a low club.
