East's X is a negative double, and shows 4 hearts (or more hearts and inability (<10hcp) to bid at 2 level). West 2NT should show a minimum for the 1NT rebid, so east is gambling a little bit with 3NT
The heart 6 is a poor lead. Leading from AK is safe and gives you a look a dummy to decide next move. In this case with hearts having been bid by dummy but coming down with only 4 small, north would continue with the king and then the 6. South, after getting in with the JH is likely to go wrong with the defence given the poor quality spade overcall by North
