Difficult hand for partnerships with limited bidding arrangements. Experienced can have systems to avoid the above, eg 1nt 2s; 2nt 3h; 4c 5c. 3h shows 3 hearts and spade shortage, 4c is a attempt to find a minor suit fit as there is no spade stop.
You can bid 3C over 1NT when you are unsure whether to play 3NT or some other contract, With a shortage in one of the other suits I think that would be a standard response.
