4NT is RKCB. 5H shows 2 key cards without the Q trumps, so east signs off in 6H. If west had responded 5S showing 2 key cards with Q trumps, then east, with all key cards and Q trumps accounted for can bid 5NT, saying show me any kings. West bids 6C and now East can bid 7H with a reasonable amount of confidence
